autism service dog form

TLCAD does not have a waiting list (rather, an applicant pool) since the matching of the dog’s skills to the applicant’s needs, along with the matching of their personalities is very important for the long-term success of the placement. We screen and interview applicants whose needs match the strengths and skills of the dog.

TLCAD conducts Team Trainings twice a year in the Spring and Fall. When groups of service dogs become available for placement in the Spring and Fall, all eligible applicants will be notified. At that point, you will have the option to formally interview for a dog. The demand for service dogs far outweighs our supply so we cannot guarantee joining our applicant pool will result in obtaining a service dog.  Your application will remain in our applicant pool until the end of the year, at which time you can re-apply if needed for the following year. All applications expire at the end of each year and a new application will need to be completed.
